Note: This Starfighter has flown its entire life with the 3rd TFW (3rd Air Group) and at the end of its career it carried badges of 7th TFS. Currently two F-104's with code "4303 / 62-12252" can be found preserved and both are not the real ones. One of them inside the Military History Park, Ji Ji Town, Nan Tou County is actually F-104J 36-8565 "4506" and the other aircraft inside the National Science and Technology Museum, Kao Hsiung City is actually F-104J 46-8596 "4522". It is unknown why they all chose this serial for both preserved F-104J Starfighters.

There are no accidents known in which this F-104G aircraft was involved.
