3 May 1957 (YF-104A)
It crashed on Eglin AFB but was repaired. It was operational again on 6 May 1958.

14 September 1962 (QF-104A)
On this day this QF-104A was lost in the Gulf of Mexico. It was flying its second unmanned flight, which ended tragically.
